New Patient Documents

What to Expect

Please be prepared to spend approximately two hours with us for your initial consultation. This will allow time to gather a thorough medical history and to perform a full eye examination, including dilation of the pupils and, if necessary, further diagnostic testing such as retinal photography, fluorescein angiography, optical coherence tomography, or ultrasonography. After your examination, the doctor will thoroughly explain all findings, discuss your condition with you, and make appropriate treatment recommendations. Because of the comprehensive nature of this visit and because your vision may temporarily be more blurred than usual due to dilation of the pupils, we recommend that you bring a driver.

If you would like to decrease your wait time in the office, you may complete your New Patient forms ahead of time and bring them with you to your appointment.

WHAT TO BRING

  • Please bring your photo ID and medical insurance cards with you to your appointment.
  • Also, bring any prescription eyeglasses that you currently wear, a list of medications and eye drops that you are currently using, and the names and addresses of the physicians that you currently see so we may keep your doctors informed of your condition.
  • If you wear contact lenses, please bring a lens case with solution.
